PATIENT INFORMATION
New PatientOur front office staff at East Lake Advancing Dentistry is happy to assist you in scheduling appointments, completing any necessary paperwork, and will answer any billing questions that you may have.

Patient Appointments


Out of respect for all of our patients, we are able to see you by appointment only. We take great care to be thoughtful of each patients time spent in our office and we ask that you also be respectful of the business atmosphere we have created by being on time for your appointments. Understandably, emergencies arise, if you are in a situation that you cannot keep an appointment, please notify us immediately. In order to assure that no fee is assessed, we ask that you give us a minimum of 48 hour notice when canceling an appointment.

Our office hours are broad for your convenience.

Insurance & Patient Co-payments


Our front office team is there to assist you with your insurance claims and questions. Together, we can determine your benefits and the coverage available for your policy. So that we can assist you, please submit your dental insurance information along with your new patient paperwork a minimum of two business days prior to your scheduled appointment. By giving us this information ahead of time, we will be able to better assist you. We will contact your insurance company and obtain the breakdown of your benefits as assigned by your employer or that you have purchased privately.

If treatment is needed, we can provide you with a detailed plan including an estimate of expenses along with the amount your insurance company is willing to pay for each procedure, and any expenses that you will be expected to pay additionally at the time of service.

Financial Policy


Our financial policy is for the protection of our office and staff. We offer our patients the highest level of professional care and ask that you in turn maintain a responsible business relationship with us. Our financial policy is as follows:

•  Payment of the full account balance is due within 20 days of the original statement date.
•  We are able to accept payment from the following credit card companies for your convenience: Visa, Discover, American Express, or MasterCard.
•  Payment plans are available through www.CareCredit.com.

Consultations


All patients that are referred to our office from another practice, or who have self-referred themselves for a second opinion are required to undergo a complete examination by Dr. Eng before providing his diagnosis opinion. Please ensure that all available x-rays and chart notes from your prior office have been transferred to us prior to the scheduled appointment for review. Please note that many offices require 7 – 14 business days to transfer records.
